2008 OASL LIBRARY MEDIA TEACHER OF THE YEAR

APPLICATION


Please submit or have a colleague submit on your behalf the following items:

1. A one-page biographical sheet, including name, home and school addresses and phone numbers, current position and grade levels,
and information about your educational background, professional achievements and leadership roles.

2. A written narrative, not to exceed four (4) pages, which provides a description of your library media program.
Include your roles as:

a. Teacher: Describe your instructional philosophy and activities for students, staff and parents.

b. Instructional Consultant: Describe your activities in curriculum development and collaboration with teachers and others.

c. Information Specialist: Describe accessibility to materials and service. Describe how you provide and promote resources and emerging technologies.

d. Other: Describe other activities and programs of merit.

e. Goal Setter: Describe your goals for your library media program. (What's next? Start by sharing your realistic plans for next year.)

3. Include two (2) letters of recommendation from certified professional colleagues.

4. Supporting materials such as newsletters, newspaper clippings and photographs may be submitted, but such materials should be
limited to three (3) pages only.

5. Include a small snapshot of yourself.

Your application should be postmarked on or before a Date to be Announced.
